Important Reminders:

  • Deadline to apply is April 25, 2023

  • In order to apply and complete your application, Providers MUST meet with ELC staff.

  • Continue to check the ELC’s ARPA Supply Build Page “Help Desk” tab for updates.

 

The following information is REQUIRED at the time you are completing the application AND registering staff members for any and all trainings. You MUST be prepared with the following:

  • A W-9 form is required for each staff member (that is registered for a training/course)

  • W-9 forms are available in the Webauthor (application) platform or IRS website.

  • Staff members’ W-9 information MUST agree with the information in Webauthor.

  • Staff members MUST sign in person with the Provider representative or via their mobile phones if requesting the Recruitment or Health and Safety Bonus.

  1. Staff LEGAL name

  2. Staff Social Security number

  3. Staff Date of Birth

  4. Mailing Address – MUST AGREE with the W-9

  5. Personal email

  6. Personal cell phone

  7. Position Title

  8. Position Start Date

  9. Age Group they are teaching

  10. Years in the Early Childhood Field

 

 

***If applying for the Health & Safety Bonus, a DCF Transcript reflecting 45-hr training is completed with a YES for Part I and II, and  5-hr Early Literacy will be required.***
