Approved Providers can then submit requests for your Educators and child care site
In order to apply and complete your application, Providers MUST meet with ELC staff.

The following information is REQUIRED at the time you are completing the application AND registering staff members for any and all trainings. You MUST be prepared with the following:
A W-9 form is required for each staff member (that is registered for a training/course)
W-9 forms are available in the Webauthor (application) platform or IRS website.
Staff members’ W-9 information MUST agree with the information in Webauthor.
Staff members MUST sign in person with the Provider representative or via their mobile phones if requesting the Recruitment or Health and Safety Bonus.
